The role you’ll contribute:
The RN Navigator serves as the primary point of contact and is responsible for coordinating the care and services of the identified patient populations across the continuum of care. The incumbent promotes effective resource utilization and assumes a leadership role with the multidisciplinary team to identify and eliminate barriers to care in order to achieve an optimal clinical outcome for the patient and their caregivers.
The value you’ll bring to the team:
Demonstrates the knowledge, skill and coordination to provide nursing care and guidance to the patient from first contact and throughout the continuum of care.
Guide patients on treatment plans, next steps in appointments (i.e. surgical prep, imaging, labs, tests, which provider to see next , etc.)
Assist with scheduling follow-up appointments and provide information and reminders to patients & physicians offices.
Ensures an extraordinary customer experience.
Track outcomes of referrals and treatments.
Maintain detailed records to measure effectiveness of program and suggest ways to improve throughout the process.
