Job Summary

Under the direction of the service line Director and according to hospital policies and procedures, the Navigator facilitates timely, integrated and coordinated approach to the care of patients.  Uses a multi disciplinary approach and acts as a liaison between the patients, staff, and referring physician offices as well as ancillary areas of the hospital system.  Identifies learning needs and the content for educational programs.  Develops, presents, coordinates and evaluate education activities.  Effectively performs in the role of clinical, facilitator, advocate, consultant and/or researcher.

Education

Required:  4 year Bachelor Degree Nursing

Combination of post secondary education and experience will be considered in lieu of degree

Licensure/Certifications

Required: KY Registered Nurse License, Basic Life Support

Minimum Qualifications

Minimum Years & Type of Experience:  Minimum of two years clinical nursing experience with demonstrated clinical competence in patient care, teaching and management of patients.
