Summary
The Neuroscience Coordinator is responsible for the organization of services and systems necessary for a multidisciplinary approach to the care of neurological and neurosurgical patients across the continuum of care. The program coordinator will utilize professional guidelines, regulatory standards and evidence-based practice to support the service line and ensure quality patient care and optimal patient outcomes. The program coordinator functions as a member of the multi-disciplinary team and is responsible for working collaboratively with the medical staff, clinical staff, ancillary services, and hospital leadership. The coordinator will provide expertise, leadership, and coordination for stroke program accreditation.
Essential Duties and Responsibilities
These duties and responsibilities described below represent the general tasks performed on a daily basis; other tasks may be assigned.
Plans, organizes, and evaluates neurological/stroke patient care to ensure compliance with regulatory requirements, accreditation standards, and national guidelines for patient care management. Monitors performance improvement indicators (i.e. core measures, Get with the Guidelines, accreditation metrics) to identify both clinical and systematic opportunities for improvement. Develops and leads performance improvement initiatives. Incorporates data and information to continually improve operations within the service line. Develops and maintains educational neuro/stroke programs both for the hospital and the public. Serves as a liaison and leader for collaboration with other care systems (i.e. EMS, other hospitals, community organizations). Develops, leads, or participates in organizational and regional committees necessary to support neuro program function. Develops policies and procedures to support neuro program function. Fulfills accreditation obligations (i.e. data submission, application completion, and site visit participation). Collaborates with the Stroke/RRT Nurse Manager to guide and evaluate the stroke care provided by the Stroke/RRT team. Minimum Required Education: Must be a Registered Nurse licensed in the State of Virginia or compact state. BSN required. Must be data and computer proficient.
Preferred Education: MSN preferred
Experience: Two years stroke/neuro program experience including stroke program accreditation and clinical operations.
