Position Purpose
Nurse Family Partner - Nurse Home Visitor is responsible for providing comprehensive nursing services to women and their families eligible for the NFP Program. Nurse home visitors are responsible for maintaining the highest standards in clinical nursing practice and adherence to the NFP model, and to policies, procedures, guidelines, and standards of NFP and of the lead agencyWhat you will do:
Completes all required NFP education.Attends and participates in NCAST Feeding and Teaching Scales trainingDevelops therapeutic relationships, utilize concepts of reflection and motivational interviewing with women and their families in a home visiting environmentAttends and participate in bi-weekly team meetingsParticipates in quality improvement effortsMaintains confidentialityDocuments appropriatelyMinimum Qualifications:
Education: Bachelor’s degree in nursing requiredCurrent RN licensure in the state of FloridaCurrent CPR certificationBilingual preferredExperience: Two years recent experience in maternal/child health, public health, home visiting or mental/behavioral nursing preferredHome visiting experience preferredBasic computer skillsMust be able to work a flexible schedule including some evenings and weekends (per agency requirements)Able to travel by air to Denver, CO for all face-to-face education sessionsAbility to work in a collaborative team environment.Must be comfortable working and serving in a diverse and inclusive environment, and operating in a collaborative, shared leadership environment.Must possess a personal presence that is characterized by a sense of honesty, integrity, and caring with the ability to inspire and motivate others to promote the philosophy, mission, vision, goals, and values of Trinity Health.Position Highlights and Benefits
