RN Patient Navigator - Bariatric Surgery Clinic

GENERAL DUTIES

Ensures delivery of high-quality patient care including assessment, planning, intervention/evaluation, and monitoring completion of patient education and discharge planning

Participates in the day-to-day operations of the department, under supervision of nursing manager/director. Acts as a clinical and operations resource person maintaining current knowledge of clinical practice and departmental operations.

Plans daily staffing coverage according to department needs and appropriate skill mix. Adheres to the staffing plan and monitors staffing level on a daily basis. Modifies staffing patterns based on patient acuity, current technology, and available resources.

Maintains employee accountability with all hospital policies and procedures.

Inspects department and completes routine environmental rounds. Completes safety rounds as prescribed by organizational safety plan. Completes regular rounds on all patients to assure that needs are met and resolves any issues that are identified. Monitors compliance of core measure initiatives for all appropriate patients. Participates in chart review to evaluate compliance with quality standards. Participates in monitoring quality control standards for the department.

Participates in cross-organizational activities to improve organizational performance.

EXPERIENCE QUALIFICATIONS

1 year of professional nursing experience in relevant specialty area (medical/surgical, intensive care, emergency, perioperative, etc.).

EDUCATION QUALIFICATIONS

Graduate of an Accredited Nursing Program

Required: Associates Degree

Preferred: BSN

LICENSES AND CERTIFICATIONS

Valid license or temporary permit to practice professional nursing in Louisiana. American Heart Association BLS-HCP.
