General Summary:
Reporting to the Service Line or specialty program leader (Neuroscience), the Patient Navigator serves as the primary point of contact for the patient through the continuum of care; diagnosis, planning, treatment, pre and post-operative care, discharge and follow up. The Patient Navigator functions as a resource to patients, seeking patient input into the plan of care. The Patient Navigator engages and collaborates with all appropriate care givers (physicians and the entire care team) in the plan of care, while serving as the advocate for the patient. The Patient Navigator provides patient and family education, and access to hospital and community resources as well as community education and screening.
What you will need:
Registered Nurse with a Bachelor of Science Degree in nursing or completion of an accredited Physician Assistant program required.Current Illinois licensure to practice nursing or current Illinois Physician Assistant license required.Three (3) years nursing or Physician Assistant experience in assigned specialty field.Benefits:
