**Position Profile**
The outbound Bilingual Phone RN initiates phone calls to patients upon discharge from the hospital to determine the patient's experience and to provide discharge teaching. Scope of practice in this role is provided solely via telephone consultation. This position is base out of our Central Clinical office located in Phoenix.
**Responsibilities**
§ Manages phone calls effectively and efficiently.
§ Processes timely and accurate patient status reports.
§ Collaborates in providing patient care.
§ Creates timely and accurate documentation.
§ Utilizes resources effectively and efficiently.
§ Adheres to scope of practice/HOV standards and facilitates continuously improved processes/services.
§ Maintains and enhances professional skills.
§ Adheres to high standards of personal and professional conduct.
**Minimum Qualifications**
§ Bilingual – English/Spanish
§ Graduate of accredited school of nursing.
§ Current Arizona RN licensure in good standing.
§ Minimum one year of recent experience in medical, surgical or critical care as a professional nurse.
§ Strong clinical assessment skills.
§ Excellent communication and interpersonal skills.
§ Solution driven, creative and resourceful problem solving skills.
§ Ability to manage time well and perform assigned duties with attention to detail, accuracy and follow-through.
§ Proficient computer skills.
§ Minimum two years of recent experience in medical, surgical or critical care nursing.
