Eddy SeniorCare has a rewarding opportunity for Registered Nurse to work in our Latham Clinic!

PACE is a unique program on the forefront of trends in healthcare, combining an insurance role (we are the Medicare/Medicaid insurer for our participants) with a comprehensive healthcare provider role (primary care, home health, adult daycare, transportation, all coordinated by an interdisciplinary team). The focus of a PACE program is on the prevention of medical issues in order to allow the participants to safely stay living in the community.

As a Registered Nurse in our clinic, you will play a vital role in participant care, supporting our physician team and interacting with our Interdisciplinary team of nurses, social workers, therapists and dietitians. This position is ideal for a nurse who thrives in a team-oriented environment and enjoys working with aging adults.

Physicians in the PACE clinic average 3-6 visits per day, having at least 30 minutes for acute visits and 60-90 minutes for assessments! This is not a clinic or program that focuses on the number of participants that are seen in a day.

Key Responsibilities

•Provide direct patient care, including assessments, vital signs, and administering medications Educate patients and families on health conditions and care plans Maintain accurate and timely documentation in electronic medical records (EMR) Collaborate with physicians, clinic staff and interdisciplinary team to ensure coordinated care Assist with triaging medical issues, monitoring medication workflow and tracking required participant documentation

Required/Preferred

Must be a graduate of an accredited Nursing Degree program. BSN preferredMust possess a current, unencumbered, active license to practice as a Registered Nurse in the State of New York. Valid driver's license in good standing and proof of insurance required. Prefer minimum of one (1) year of documented experience working with a frail or elderly population


Pay Range:$35.00 - $47.75

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
