Overview Catholic Health Home Care is looking for a RN Project Manager for our Telehealth Program. On-site role in our Farmingdale office Monday-Friday 8:00 AM to 4:00 PM The Telehealth RN Project Manager ensures that high-quality nursing care is delivered remotely, coordinating with patients and healthcare providers to assess, plan, and deliver care in an efficient and patient-centered manner. Helping patients proactively manage their chronic diseases. The Telehealth RN Project Manager will also be responsible for the supervision, training, and development of telehealth nurses, while ensuring compliance with agency standards of practice. Job Details The RN Project Manager for Telehealth is responsible for: Direct Supervision of Team including 9 RNs, 4 Techs and a Team Assistant. Building and evaluating team members and hold clinicians accountable to established agency standards of practice, documentation and productivity Evaluating clinical competency through direct observation, skills assessment, and performance appraisals Supervising compliance with timely submission of all documentation Overseeing daily remote patient monitoring by Registered Nurses Overseeing equipment deployment by Telehealth Technicians Providing external customer service to system hospitals and community partners Collaborating with system entities for evidenced-based practice outcomes Holding staff accountable to the following productivity standard Onboarding newly admitted patients by Registered Nurse Providing supportive education to patients in addition to skilled nursing visits Providing efficient deployment of devices by Telehealth Technicians to exemplify optimal patient/caregiver satisfaction Troubleshooting equipment issues as needed. Minimum Qualifications Previous leadership experience Baccalaureate degree in Nursing (BSN) required, with Masters degree (MSN) preferred. Salary Range USD $125,000.00 - USD $140,000.00 /Yr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
