Are you passionate about behavioral health and ready to make a difference in the lives of patients? We are seeking a dedicated Behavioral Health Registered Nurse to join our team and assist in coordinating care referrals and ensuring patients receive the best possible care transitions. Full time opportunities are currently available. About the Role: As an RN in our Behavioral Health Team, you will: Collaborate closely with our care coordination team to facilitate referrals to other facilities or higher levels of care. Play a vital role in arranging final disposition arrangements, ensuring seamless transitions for patients. Utilize your expertise in behavioral health to provide compassionate and effective care to patients in need. Responsibilities Assessment: Ability to effectively assess patients holistically for behavioral health needs, work with physicians for plan of care , transfer and discharge planning needs, real and/or potential. Monitors for return patients for appropriate outside care management. Planning: Ability to use resources effectively to insure timely transfer/discharge of patients to the appropriate level of care. Refers to local agencies as needed. Collaborates with state agencies. Communicates with nursing and physcians on plan of care. Maintains familiarity with laws, regulations and interpretations pertinent to discharge planning. Understands local resources. Uses tele-psych equipment, paper work required for BH transfers and documentation in patient record. In collaboration with nursing and other team members, charts ongoing assessment, planning, implementation and evaluation of outcomes of discharge planning. Maintains a resource folder for all units for BH. Decision Making: Ability to make sound decisions regarding outpatient resources in collaboration with medical provider. Organizational Skills: Ability to prioritize cases and complete tasks in a way to expedite discharges/transfers. Patient teaching activities. Works with Managers of all units for staff education for BH patient population. Other duties as assigned. Qualifications 1 year behavioral Health Experience. Skills include ability to complete psychosocial assessment, exceptional communication and problem solving skills, general knowledge of utilization management, understanding of levels of care and community resources. Demonstrated skills of organization, team work, strong interpersonal and written communication skills, and flexible work schedule, and be self motivated. Must be proficient in word processing programs and use of clinical software. BSN preferred. Certification for Delaware Commital Process preferred. Credentials Essential: * RN - Registered Nurse * BLS - Basic Life Saving certification Education Essential: * RN Diploma/RN Graduate Program Entry USD $36.20/Hr. Expert USD $59.73/Hr.
