RN / Registered Nurse - SANE 2 (Sexual Assault Nurse Examiner) - Forensic Program - Per Diem - Variable

REPORTING RELATIONSHIPS/SUPERVISORY RESPONSIBILITIES

 Reports to forensic coordinator. This nurse leads the SANE team members in the care as directed by the International Association of Forensic Nurses (IAFN), program medical director, and in collaboration with attending providers and other healthcare providers in accordance with the philosophy of ProMedica Acute Care Hospitals.

 POSITION SUMMARY

The SANE Registered Nurse provides timely accurate collection of forensic evidence in sexual assault cases. This includes sexual assault/abuse collection kits for male and female patients. The SANE nurse actively participates in the creation of an environment that fosters patient, family, physician, and employee satisfaction. The SANE nurse provides nursing care to individuals and groups who require specialized knowledge, judgment, and skills derived from the principles of biological, physical, behavioral, social, spiritual/cultural, and nursing sciences. The SANE Registered Nurse functions within the full scope of nursing practices as noted by the Board of Nursing of their designated state for which they are practicing in addition to compliance with any of the hospitals accrediting bodies. The SANE nurse will provide coverage to the following North and South region ProMedica Facilities: Monroe, Hickman, Fremont, Fostoria, Toledo, Flower, and Bay Park.

ACCOUNTABILITIES

 *All duties listed below are essential unless noted otherwise*

 1. SANE Specific Responsibilities

 a. Demonstrates the ability to work collaboratively with local domestic violence and sexual assault advocates, law enforcement agencies, social work, and emergency department personnel.

b. Performs forensic examinations and collection of evidence in accordance with state and professional organization guidelines.

 c. Demonstrates competence in the use of additional evidence collection tools such as colposcope, Bluemaxx light, toluidine blue, and photo documentation.

 d. Completes SANE documentation to include consents, discharge instructions, assessment, and interventions.

e. Initiates appropriate referrals for follow up after care as needed.

 f. Performs quality assurance functions and other duties as assigned by the program coordinator.

g. Attends a minimum of two (2) regional/departmental SANE meetings and education events as scheduled.

 2. Patient & Family Centered Care

 a. Recognize the patient or designee as the source of control and full partner in providing compassionate and coordinate care based on respect for patient preference, values, and needs.

b. Elicit and communicate patient values, preferences, and expressed needs.

 c. Provide patient-centered care with sensitivity and respect for the diversity of human experience in collaboration with the interdisciplinary team. Engage patients or designated surrogates in active partnerships that promote health, safety and well-being, and self-care management.

d. Addresses patient family education needs considering patient level of comprehension, style of learning from the point of entry through discharge.

e. Navigates and collaborates with patient, family and healthcare team regarding the care provided and needed to ensure appropriate transition along the continuum.

 f. Evaluates patient’s response and interventions to ensure optimal patient care.

3. Evidence-Based Practice

 a. Integrate best current evidence with clinical expertise and patient/family preferences and values for delivery of optimal health care.

b. Base individualized care plan on patient values, clinical expertise, and evidence.

4. Nursing Practice

a. Assesses, formulates nursing diagnosis, plans, and implements and continually evaluates effectiveness of the individualized written nursing care plan and delivery of nursing care for each patient assigned.

b. Analyzes and prioritizes problems and makes sound judgments based on logic and/or critical thinking skills.

5. Safety and Quality Improvement

 a. Improve the safety and quality of healthcare systems by minimizing the risk of harm to patients and the healthcare team.

 b. Administers medications and executes regimens as authorized by the state licensure.

c. Incorporates quality measures and data to understand individual and system performance.

 d. Applies use of technology and standardized practices that support safety and quality.

e. Identifies strategies to reduce risk of harm to self or others.

f. Participates in unit-based quality improvement, evidence-based practice projects or research.

g. Promotes and incorporates error prevention techniques.

h. Report safety concerns and events including near misses.

 6. Informatics & Technology

a. Promote and incorporate use of technology to communicate, manage information, mitigate errors, and support decision-making.

b. Navigates, plans patient care, and documents ongoing status, interventions, patient responses, and outcomes in accurate, timely manner in the electronic health record.

c. Responds appropriately to clinical decision-making supports and alerts.

 d. Advocates protecting the integrity of the healthcare record and the privacy of each patient.

7. Communication, Teamwork & Collaboration

 a. Interacts effectively with patient, families and with healthcare team to foster open communication, mutual respect, and shared decision-making.

b. Functions as the coordinator of the patient’s plan of care through interaction and communication with healthcare team.

 f. Escalates barriers, safety and clinical concerns utilizing appropriate chain of command.

8. Leadership & Professionalism
