RN - TeleNeuro Hospitalist Nurse Facilitator - Admit Discharge Transfer Resource Nurse at Providence Medford Medical Center in Medford, Oregon.

This is a combined position that will be dedicated to two separate programs; the TeleNeuro Hospitalist program and the ADT program.

TeleNeuro Hospitalist Nurse Facilitator

Designated TelePresenter nurse to assist the neurologist with non-emergent assessments for inpatient neurological follow-up appointments. These assessments may include: NIH Stroke Scale, cranial nerves, and reflexes. Communicates with the neurologist provider to determine appointment schedule for the day and informs direct care nurse about the appointment. Moves the TeleNeuro video cart from patient room to the next patient room.

Admit/Discharge/Transfer (ADT) Resource Nurse

Facilitate patient progression by coordinating discharge planning and related tasks earlier in the patients’ stay.  Collaborate with the patients’ interdisciplinary care team to address barriers to discharge and support the patient throughout the discharge process. Float throughout the house to complete discharge related tasks for patients projected to be discharged that day or the next day. Provide discharge education to patients and their families.

0.6 FTE = 48 hours/pay period: 

Week 1:  Three 8 hours shifts (0730-1600) One 4 hour shift (0730-1130) Week 2: Two 8 hours shifts (0730-1600) One 4 hour shift (0730-1130) Weekends:  Every other weekend (Saturday and Sunday)

Required Qualifications:

Graduation from an accredited nursing program. Oregon Registered Nurse License upon hire. National Provider BLS - American Heart Association upon hire. 1 year Nursing experience.

Preferred Qualification:

National Certification in area of specialty upon hire.
