RN Transition to Practice Coach, Emergency Department - Cottonwood, AZ Job Locations US-AZ-Cottonwood Requisition ID 2025-21494 Call Required No Recruiter: ANDREW SIKORSKI Recruiter Email: Andrew.Sikorski@nahealth.com Schedule Variable Telecommute No FTE 1.000 / 40 hours (Regular Full-Time) Overview The Transition to Practice Coach is a clinical leadership role responsible for supporting the professional development and successful integration of new graduate nurses and experienced nurses transitioning to independent practice and new specialty areas. This position combines advanced clinical expertise with educational leadership to ensure safe, competent, and confident nursing practice while fostering a positive learning environment. Responsibilities Clinical Leadership & Mentorship* Serve as a clinical expert and role model for new graduate nurses and transitioning staff
* Provide direct clinical supervision, guidance, and support during orientation periods
* Assess competency development and provide constructive feedback on clinical performance
* Collaborate with unit managers and educators to customize learning experiences based on individual needs
* Support preceptors through coaching, mentoring, and professional development opportunities Educational Program Development* In collaboration with Nurse Education Specialist;
* Develop competency-based learning objectives and assessment tools
* Create educational resources, simulation scenarios, and case studies
* Facilitate group learning sessions, debriefings, and skills workshops
* Maintain current knowledge of evidence-based practices and integrate into teaching Quality & Safety Oversight* Monitor patient safety outcomes related to new nurse performance
* Identify and address practice gaps or safety concerns promptly
* Promote adherence to organizational policies, procedures, and standards of care
* Participate in quality improvement initiatives and root cause analyses
* Ensure compliance with regulatory requirements and accreditation standards Communication & Collaboration* Serve as liaison between new nurses, preceptors, management, and interdisciplinary teams
* Facilitate regular communication regarding progress, challenges, and achievements
* Participate in multidisciplinary rounds and care planning meetings
* Provide consultation to staff regarding complex clinical situations
* Document and report on program outcomes and individual progress Compliance/Safety* Adheres to regulatory and organizational standards affecting patient care and safety as well as all hospital and departmental policies and procedures. Practices in a safe, high quality and cost effective manner.
* Stays current and complies with state and federal regulations/statutes and company policies that impact the employees area of responsibility.
* If required for position, ensures all certifications and/or licenses are up-to-date and valid prior to expiration dates.
* Completes all company mandatory modules and required job specific training in the specified time frame.
* Maintains timely and accurate documentation of assessments, interventions, EMR entries, and patient responses.
* Responsible for reporting any safety related incident in a timely fashion through the Midas/RDE tool; attends all safety related training programs; performs work in a safe manner; monitors work environment for possible safety issues and ensures others are also performing work in a safe manner. Qualifications EducationAssociates Degree of Nursing (ADN) or Bachelor of Science in Nursing (BSN) - Required Master's degree - Preferred Certification & LicensuresCurrent, unrestricted RN license in state of practice
BLS and ACLS certification (additional certifications as required by specialty area) ExperienceMinimum 2 years of clinical nursing experience in relevant specialty area
2 years of clinical experience and 1 year of preceptor or mentor experience
Previous experience in nursing education, staff development, or clinical leadership preferred
