We are seeking a Registered Nurse (RN) Transitional Care Case Manager to:

Bridge inpatient to ambulatory care for medical, surgical, and trauma populations. Coordinate with interdisciplinary teams to ensure remarkable healthcare.  Use clinical expertise to empower patients during post-acute hospitalization recovery.
