We are seeking a Registered Nurse (RN) Transitional Care Case Manager to:
Bridge inpatient to ambulatory care for medical, surgical, and trauma populations. Coordinate with interdisciplinary teams to ensure remarkable healthcare. Qualifications Bachelor's Degree in Nursing Required Master's Degree in Nursing or Health Care related field Preferred Work Experience 3 years of relevant clinical nursing experience. Equivalent combination of education and experience will be considered Required Surgical, medical, trauma, and intensive care experience Preferred Licenses & Certifications Licensed as a Registered Nurse (RN) in the State of WI or holds a license issued by a jurisdiction that has adopted the nurse licensure compact Upon Hire Required RN licensure to practice in the State of Illinois within 6 months Required Current CPR/BLS Certification Upon Hire Required ACMA certification as a case manager Preferred Our Commitment to Diversity and Inclusion UW Health is committed to being a diverse, inclusive and anti-racist workplace and is an Equal Employment Opportunity, Affirmative Action employer.
