The Triage Nurse (RN) assesses patients’ health problems and needs, develops and executes nursing care plans, and assists in maintenance of patient medical records. The Triage Nurse (RN) work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.

Job Description
Identifies patient care needs by establishing personal rapport with patients and employs a variety of strategies, approaches, and techniques to manage a member’s physical, environmental, and psycho-social health issues.
Responsible for evaluation of patients with repeated ER/Hospital diagnosis and high-risk chronic diagnoses initiating short term quality care plans, care coordination and/or patient education collaborating with the care team and clinicians following nursing division’s philosophies and standards of care set by state board of nursing, nurse practice act and other governing agency regulations.
The high acuity nurse will work in partnership with the center’s clinicians, front office staff, care integration team and other team members to anticipate and resolve patient needs.
Promotes patient's independence and health care literacy by establishing patient care goals; teaching patients, family and caregivers to understand condition, medications, and self-care skills; answering questions.
Helps coordinate and facilitate High-Risk patient round meetings for the centers assigned. Presenting patients and coordinating follow up plans.
Assist patients in identifying and resolving barriers that hinder effective care.
Understands organizational, market and center level operating goals and metrics.

Prior clinical experience in case management a plusIntermediate knowledge of Microsoft Word, Outlook, Excel and prior EMR exposure plusBilingual is a plusScheduled Weekly Hours
40Pay Range
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
