**Overview** Mercy Medical Center Mt. Shasta is a member of the Dignity Health system of health care providers and is a not-for-profit medical facility. We are designated as a 25 bed Critical Access Hospital and are accredited by the Joint Commission on Accreditation of Health care Organizations. For more information visit www.mercy.org. **Responsibilities** Under direct supervision the incumbent functions independently in performing the nursing process through assessment, planning, implementation and evaluation of safe therapeutic care for patient overt and covert needs and/or complications of treatment in the patient's home. Incumbent is also responsible for providing assistance to physicians and other members of the team in coordination of services and resources for high risk and chronic care patients. To provide education and supportive intervention aimed at increasing patient knowledge and confidence in managing their health as well as to provide coordination of services across the continuum. Will have a strong medical background to assist in this process, is knowledgeable of community resources and is able to address psychosocial issues that affect patient's care. **Qualifications** **Requirements:** One year in a Clinic Setting, One year experience Acute Care experience One year experience Home Health/Public Health experience **Special Skills:** Wound Care, Working knowledge of Google work space, Infusion Therapy Computer experience, exemplary telephone etiquette and interpersonal communication skills **Education:** AA degree in nursing required **Licensure:** California RN License BLS-CPR California Drivers License Automobile Insurance **Preferred:** Case Management, Care Coordination education or other related fields.
