We are seeking a dedicated and empathetic Home Health Nurse to join our team. As a Home Health Nurse, you will play a crucial role in delivering high-quality healthcare services to patients in their homes. You will work closely with patients, their families, and other healthcare professionals to ensure comprehensive and coordinated care.
Key Responsibilities:
+ Provide skilled nursing care to patients in their homes, including wound care, medication administration, and monitoring of vital signs.
+ Develop and implement individualized care plans based on patients' needs and medical conditions.
+ Educate patients and their families on disease management, medication usage, and lifestyle modifications.
+ Collaborate with physicians, therapists, and other healthcare professionals to ensure continuity of care.
+ Conduct regular assessments and adjust care plans as needed to achieve optimal patient outcomes.
+ Maintain accurate and timely documentation of patient care and progress.
Qualifications:
+ Valid Registered Nurse (RN) license in California.
+ Minimum of 1 year of nursing experience in home health.
+ Strong clinical skills and knowledge of home health care practices.
+ Excellent communication and interpersonal skills.
+ Ability to work independently and manage time effectively.
+ Compassionate and patient-centered approach to care.
