Job Summary:

Primary function is to process all aspects of scheduling nursing visits via Home Care Home Base software system.

 Functions:

Schedule and appropriately document patient schedules in a timely manner. Participate in coordinating care with management and patient interdisciplinary team. Provide effective communication to patients, staff members, other health care professionals and referral sources. Respond to agency and patient needs in a professional and creative manner. Demonstrate commitment, professional growth and competency. Promote Agency philosophy and administrative policies to ensure quality of care.

Skills & Experience:

Minimum 1 year of home health experience. Excellent computer skills required. Excellent interpersonal and organizational skills. Basic knowledge of scanner/fax/copier equipment. Phone etiquette skills required. Ability to take direction with a multitude of tasks. Typing ability 40-50 WPM. Knowledge of Medicare regulatory guidelines.

A reliable source of transportation and valid/current auto liability insurance is required.
