PURPOSE OF POSITION
To coordinate/assign visiting disciplines to patients including but not limited to: nursing, physical therapy, occupational therapy, speech language pathology, medical social services, and home health aides. Coordinates schedules between multiple providers and disciplines to assure optimal patient care, efficient utilization of services, and compliance with physician orders.
RESPONSIBILITIES & EXPECTATIONS
Coordinates scheduling of homecare patients with assistance of the Clinical Managers Responsible for maintaining current and accurate schedules within the scheduling system for use by staff Updates direct care staff schedules as needed throughout the day Coordinates with Intake to schedule all patient admissions Coordinates initial visits for internal multidisciplinary referrals Works under the direction of the Clinical Manager to revise schedules for call-offs, conflicts or high utilization of service Ensures prompt scheduling of emergent/as-needed visits as directed by the Clinical Manager or Resource RN Assures timely revision of staff schedules on a daily basis with all final and accurate schedules completed and available to staff no later than 4 pm Monday through Friday Schedules visits to clinicians to promote efficiency of operations considering location, travel time, level of provider required and status of the employee (full time, part time, casual) Promotes continuity of clinical field staff when scheduling patient visits Utilizes the Electronic Medical Record (EMR) for scheduling Maintains confidentiality with employee and company sensitive information Acts as a liaison between patients and staff related to patient schedules. Maintains excellence in customer service and telephone etiquette. Documents patient contacts in the EMR Consistently communicates with all patients, families, customers, and other personnel in a professional, respectful, and courteous manner Facilitates communication by sending information to the case managers, managers and admitting clinicians the day the information is received. Provides information related to admissions to Insurance verification daily Utilizes the scheduling task list to identify visits that need to be scheduled, including starts and resumptions of care Daily, reviews start of care, resumption of care and recertifications for appropriate changes in admission status, appropriate scheduling of visits in accordance with orders and the presence of necessary scheduling notes Facilitates coordination of care by sending email notification to case managers, home care clinical team and assigned clinicians for starts and resumptions of care the day information is received Each day, schedules unassigned visits with clinical direction from the Clinical Managers as needed Proactively manages future schedules, taking into account staff with time off and other time obligations Reviews previous days schedules to identify any missed visits Schedules students and other observers as assigned Demonstrates flexibility in meeting the needs of the agency and responds positively to changes in workflow Promotes teamwork by demonstrating willingness to assist others in meeting the needs of all individuals and collaborating effectively with the interdisciplinary team and other departments throughout the organization Completes annual competency requirements Consistently communicates requests, suggestions, concerns, or problems to the appropriate personnel Consistently completes assigned responsibilities in required time frame and accepts additional duties and responsibilities as requested by Agency Director Consistently maintains a professional appearance Participates in process improvement initiatives Performs other duties as assigned Maintains Aultman standards of confidentiality, including the HIPAA Privacy Rule Creates and fosters a work environment that is consistent with the Mission, Vision and Values of the Aultman Health Foundation Supports organizational strategic goals. QUALIFICATIONS
Minimum High school graduate with previous experience working in a healthcare setting with knowledge of common medical terminology• Home care experience preferred; prior scheduling experience strongly preferred
• Able to work independently and accurately with attention to detail
• Excellent interpersonal and oral/written/telephone communication skills; strong organizational and problem-solving skills; ability to stimulate new ideas
• Proficiency with Microsoft office products including Outlook and Word
Ability to develops and maintain proficiency with the agency EMR and telephone system
WORKING CONDITIONS
The majority of work occurs during agency hours of operation with some flexibility to work before or after hours of operation and overtime to meet the needs of the agency
