Description:The primary purpose of your job position is to coordinate and schedule external appointments for all participants at the PACE Organization (PO). The scheduler will prepare schedules according to the participant needs and the participant's care plan designed by the interdisciplinary team. Works closely with the transportation department, participants, caregivers, and center staff.

Trinity Health LIFE Kinder Park a member of Trinity Health PACE, is seeking a full time Scheduler. Programs of All-Inclusive Care for the Elderly (PACE) coordinate and provide all needed preventive, primary, acute and long-term care services so that older individuals can continue living in the community. PACE is an innovative model that enables individuals who are 55 years old or older and certified by their state to need nursing home care to live as independently as possible. Through PACE, today’s fragmented health care financing and delivery system comes together to serve the unique needs of each individual in a way that makes sense to the frail elderly, their informal caregivers, health care providers and policy makers.

Many PACE participants get most of their care from staff employed by the PACE organization in the PACE center. PACE centers meet State and Federal safety requirements and include adult day programs, medical clinics, activities, and occupational and physical therapy facilities.

Position Purpose: The Scheduler at the PACE Organization coordinates and schedules external appointments for participants, ensuring that each appointment aligns with the participant’s care plan. This role involves close collaboration with the transportation department, participants, caregivers, and center staff.

What You Will Do:

Maintain accurate participant schedules, updating daily changes and ensuring all appointments are properly coordinated.Communicate with various departments, including clinical teams, to confirm appointment details and participant education.Ensure compliance with all regulations, policies, and procedures, and support a collaborative work environment.Manage appointment reminders for participants and caregivers.Assist with tracking training for agency personnel and reviewing invoices in the absence of the manager.

Minimum Qualifications:

High School Diploma or equivalent (Associate’s degree preferred).At least one year of scheduling experience; experience working with elderly or frail populations preferred.Proficiency with Microsoft Office and scheduling systems.Strong written and verbal communication skills.Ability to work independently, manage conflict, and exercise sound judgment.
