Nexus-PATH is hiring a School Case Manager for the Devils Lake, ND School District!

 Are you interested in working with children and families in a school and community setting? We believe that many children and families can benefit from the support of a committed, compassionate, and strength-based case manager.  Join our Nexus-PATH team as we expand our school case management services to the Mandan, ND area schools.    Our School Case Manager assists eligible student and their caregivers in gaining access to needed medical, social, educational and other services necessary to build resilience and overcome challenges.   NOTE: This is a 12-month (annual) position, working primarily from the school building with those students and families referred exclusively from the school district.

Responsibilities:

Provide comprehensive coordination of all tasks required to meet the treatment needs and best interests of service recipients consistent with regulatory agencies (i.e. DHS, COA)

Comply with local office procedure for referral and service delivery Complete a systemic assessment of the needs of the child and family Establish service parameters with family or legal custodian as child’s needs dictate Provide complete and required documentation, within established timelines relating to case management and treatment planning Implement treatment services to meet the needs and goals of the children and families utilizing quality case management practice to provide for the best interest of the child and family Provide direction to other team members when applicable and make recommendations for discharge, finalization and permanency Promote the needs of youth and families to ensure successful outcomes Advocate for the development and maintenance of funding and resources to meet youth and family needs Develop and maintain community contacts to promote Nexus-PATH and collaborative efforts Provide crisis intervention as needed to maintain safety and provide for the best interest of the child and family

Trauma-Informed Care

Integrates trauma informed care principles into practice by sharing knowledge and understanding of trauma with the child, family and relevant team members  Utilizes and advocates for the use of trauma-sensitive strategies to support the youth and their family Routinely screens for trauma exposure and related symptoms and demonstrates competence in using screening tool results to enhance service provision Routinely makes resources available and shares knowledge with foster families and providers on trauma exposure, its impact, and trauma-focused treatment. Engages in efforts to strengthen the resilience and protective factors of children and families impacted by and vulnerable to trauma. Emphasizes continuity of care and collaboration across child-service systems. Understand the potential for secondary traumatic stress and practices self-care strategies to mitigate these effects.

Meet standards for productivity and caseload size while maintaining appropriate documentation and files to support the services that are provided to families

Participate in staff and supervision meetings and required Nexus-PATH in-service meetings

Ensure confidentiality of all treatment and services information

Qualifications:

Bachelor’s degree in Social Work preferred; will consider Bachelor’s degree in a human services related field 2-3 years of work experience serving children and families Current North Dakota social work licensure preferred Experience in family-based service strongly preferred Valid driver’s license required.  Must meet state regulating agency driving requirements, maintain auto insurance and pass corporate insurance eligibility standards.
