About the Program TheSchool Based Early Support Program(SBES) of Graham Windham is a new family support model that focuses on providing support to families whose children are served within the Bronx community school district 8 (Hunts Point). The program enhances partnerships between schools and community providers, offers case management, facilitates parent-support and education groups, and helps families with their basic needs. About the Position The School Outreach Liaison has the primary responsibility to develop effective partnerships and relationships within and external to the school; to create and nurture a network of resources that can provide support and services to families. The Liaison promotes the services and supports offered by the SBES Program and the host schools, working with the school community and across Graham programs to identify families who may benefit from the SBES services. Using a strengths-based Appreciative Inquiry lens, and grounded in the motivational interviewing and solution-based casework models, the Liaison builds strong and productive connections with school personnel, community and agency partners, and families. Duties and Responsibilities : + Family Support and Empowerment: + Coordinate and facilitateTriple P Positive Parenting Groupusing the evidence-based Triple P Model that empowers parents by teaching effective techniques to manage challenging behaviors and build strong relationships with their children. + Supervise the Community Pathway Worker: Provide guidance and support to the Community Pathway Worker, ensuring they effectively connect families with needed resources and services. + In partnership with Program Director, facilitate a co-design process with all three schools in the development of school-based offerings. + Conduct Success (Home) Visits to families as needed. + Complete required documentation on family’s engagement. + Develop School-Based Offerings: Identify the needs of the school community and create engaging offerings such as workshops (parenting skills, financial literacy), cultural events celebrating diversity, and youth events promoting social and emotional learning, in collaboration with respective schools, and other Graham programs. + Build a strong and supportive school community: + Serve as a trusted liaison between 3 partner school staff, families, internal Graham programs and community partners to coordinate outreach, services and school events. + Proactively build trust and rapport with families from diverse backgrounds, promoting inclusivity and accessibility. + Meet with school teams weekly to identify students that can benefit from supports offered by SBES team. + Educate school staff about the program and its target population. + Assist with school program events and initiatives focused on supporting families within the school community. + Develop and Maintain Communication Channels: Keep all stakeholders informed through regular communication, including newsletters, social media, and meetings. + Work collaboratively with Program Director, Supervisors, Case Planners, Parents and other program staff. + Attend weekly partner school based meetings and share offerings. + Attend weekly program staff meetings, supervision, and trainings. + Build relationships with community-based service providers and other key community stakeholders. + Join forces with all Graham programs to ensure an integrated approach to serving families. + Program Management and Quality Improvement: + Collaborate with program QA/QI team, Program and stakeholders (ACS and others) to ensure program quality and effectiveness. + Track program participation, outcomes, and data in agency database to maintain accurate records and generate reports. + Continuously evaluate program effectiveness using data to identify areas for improvement. + Implement data-driven improvements and stay up-to-date on best practices in family support and child development. + Stay Current on Family Support Best Practices: Attend relevant trainings and workshops to continuously improve program effectiveness. + Including but not limited to Motivational Interviewing, Appreciative Inquiry, Trauma informed care, cultural competency and other workforce development trainings. Qualifications Qualifications: + Master’s Degree in Social work, education or similar human services field required + Bilingual English/Spanish required + Supervisory and or coaching experience. + Minimum 2 years of experience working with families and children. + Proven experience facilitating parenting programs. + Excellent communication, interpersonal, and organizational skills. + Ability to multi-task and manage multiple sites. + Regular travel among all 3 schools located in the district. + Strong understanding of child development and family dynamics. + Possess a strong belief that families have the ability to grow and change. + Familiarity with community resources and support systems (preferred). + Experience working in a school setting (a plus). + This is a full-time, in-person position.
