SCSC Transition of Care Specialist

Pay $40,000 - $45,000 annually

The Transition of Care Specialist (TOCS) supports the Crisis Stabilization Center (CSC), and the continuum of crisis services for Community Health and Behavioral Services (CHBS). The TOCS is responsible for processing and maintaining discharge records and referrals for clients in a manner consistent with medical, administrative, ethical, legal, and regulatory requirements of the health care system. The TOCS supports the daily operational functions and assists members of the CSC team to promote a smooth working environment. 

 

Core Responsibilities 

Ensure 100% accountability for all electronic health records (EHR). Collaborate across systems, internally and externally, to promote effective communication and collaborative efforts.  Sends out discharge summaries for all patients who have identified outside providers or collaterals. Process requests for follow up visits for all patients receiving referrals as a part of discharge planning. Responds to requests for copies of health records; ensures appropriate paperwork (release of information, subpoena, etc.)  Educate individuals on referred services and ensure a high level of individual satisfaction through proactive support and active listening. Coordinate transitions of care as the referrals to both internal and external providers. Process, store and retrieve electronic health information. Maintains and utilizes a variety of health record indexes, storage and retrieval systems. Referral monitoring, follow up contacts with agencies and patient to assure referral appointments were attending. Scans patient health records into the patient’s electronic file and keeps electronic files current. Performs other duties as assigned and assist all crisis staff as needed. Maintain compliance with Regional Health Information System (RHIO). Knowledge of HIPAA regulations.

 

Qualifications

High School Diploma or GED equivalent required. Associates degree in a relevant field preferred. Strong customer service skills with experience working with individuals with intense needs Certain deadlines and unanticipated developments may require work during evenings, weekends or holidays. Must have a valid NYS Driver’s License
