Position Summary:
To provide Self-Directed Support Brokerage and Fiscal Intermediary Services (Coordinator) to individuals who have chosen Self- Directed Services (SDS). Primary duties include assistance in developing a Self-Directed Budget and related Staff Action Plan(s), tracking of SDS budgets, prompt billing and payment of approved services, Medicaid and corporate compliance adherence and employer of record duties for all “self-hired” staff. Services provided through these plans allows individuals to live, work and grow in the most appropriate, integrated settings designed by them and their Circle of support (COS) team.
Primary Duties and Responsibilities:
+ Assist with development and maintain a Circle of Support (also known as a planning team) and assist in directing planning meetings.
+ Assist in developing, updating and maintaining a Self-Directed Budget and related Staff Action Plan(s) that is consistent with the Life Plan.
+ Maintain, review and submit billing documents ensuring that they meet OPWDD, Medicaid and Department of Labor standards.
+ Accurate maintenance of caseload records in primarily Tioga and Tompkins Counties.
+ Assist to hire and retain appropriate support staff including recruiting, interviewing, hiring, scheduling, payroll processing, and supervising of staff.
+ Assist with processing New Employee Orientation paper work and training in addition to monitoring annual training required by staff.
+ Monitor financial statements for expenditure issues.
+ Assist with community inclusion.
+ Supports the philosophy, goals and policies of Springbrook to the individuals, family of the individual, personnel , and the DDRO Liaisons in a positive and constructive manner
+ Address policy and procedure questions from staff, individuals and families
+ All other duties as assigned
Qualifications, Skills and Knowledge Requirements:
+ Bachelor Degree’s required (Human Services preferred, others will be considered)
+ Preferred (1) years’ experience working with individuals with developmental disabilities or related field.
+ Must be able to work a flexible schedule.
+ Duties require professional verbal and written communication skills.
+ Proficiency in or knowledge of using a variety of computer software and e-mail applications, especially Microsoft Excel, Outlook and Word; have the aptitude to learn other computer software as necessary.
+ Must be able to lift 50 lbs. independently and assist in lifting weights exceeding 50 lbs.
+ Meet the requirements to drive Springbrook vehicles as required.
+ Access to reliable transportation as travel is required.
+ New York State Driver’s License is required.
