In accordance with the Vision, Purpose, and Values, and strategic direction of the Vancouver Island Health Authority (Island Health) patient and staff safety is a priority and a responsibility shared by everyone; as such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.

Working in the community setting and as part of a multidisciplinary team, the Geriatric Outreach Nurse acts as a special resource in psychogeriatric care, including concurrent disorders; provides consultation and formal teaching; is responsible for the provision of clinical services to the psychogeriatric client through assessment, treatment, monitoring and directing care; act as liaison with community agencies and care providers; and assists in community development activities.

QUALIFICATIONS: Education, Training And Experience

Graduation from a recognized nursing program or evidence of an equivalent combination of education, training and experience. Three (3) to five (5) years recent, related experience, including two (2) years relevant clinical experience. Registration with BC College of Nurses and Midwives as a practicing RN registrant or a practicing RPN registrant. Knowledge of current theories, technologies and practices in the psychogeriatric care area; of applicable legislation, such as the Mental Health Act and Adult Guardianship Act; and of adult education principles. Valid BC Driver's License.

Skills And Abilities Ability to develop and integrate educational strategies and programs and to teach; Ability to function independently and work collaboratively within a team and demonstrate strong leadership abilities; Effective oral and written communication skills; Effective interpersonal and facilitation skills; Ability to organize, plan and prioritize own work and that of others; Ability to use a collaborative approach to problem solving supported by a systems approach; Ability to effectively employ human relation skills, including conflict resolution; Demonstrated contribution to the professional and the organizational vision as it pertains to psychogeriatric clinical practice and patient/client care; Demonstrated critical and analytical thinking, decision making, and problem solving skills; Demonstrated commitment to collaborative practice and life long learning; Computer skills, including the ability to use MS Office and other related computer technology; A sound understanding of current and future nursing and health care issues and the potential impacts on nursing education and clinical practice; Ability to operate related equipment; and Physical ability to perform the duties of the position.
