Under the administrative direction of the Coordinator, and a part of the therapeutic team under the Clinical Director, the Case Worker is responsible for providing primary therapeutic, case management and mental health support services for assigned clients, whether they are in hospital, residential or community settings. Such services are provided where and as required, in accordance with care plans agreed to by the multidisciplinary clinical team, the individual and family.
QUALIFICATIONS: Education, Training And ExperienceRegistration with BC College of Nurses and Midwives as a practicing RN registrant or a practicing RPN registrant. Two years of recent related experience. Valid Class 5 vehicle license.
Skills And Abilities Clinical understanding of affective disorders, schizophrenia and other serious psychiatric disorders and their treatment, using a comprehensive bio psychosocial model of care. Ability to work in an outreach case management modality. Ability to work effectively as part of a multi-disciplinary team. Ability to deal effectively with a wide range of contacts. Ability to organize work effectively, establish and carry out priorities to meet critical deadlines. Ability to communicate effectively, both verbally and in writing. Additional information for applicants This casual position will be part of an exciting new MHSU hub concept in the western communities where a team-based care model will be used to meet the identified needs of clients. The hub building is set to open in 2024 and until that time, the team will operate virtually and at Westshore CARES (582 Goldstream Ave, Unit 206). During the transition, remote/flexible work arrangements may be available. All applicants must be able to work in the new hub location once it opens. This new service will feature a dynamic, collaborative team made up of counsellors, case managers, nurses, concurrent disorders workers and addictions medicine services. The service will include a single-session walk-in access point for MHSU services. The goal is to create more accessibility and responsiveness for people living in the Western communities. The MHSU hub will also feature psychiatry services including the Enhanced collaborative care model, and follow-up psychiatry for hub clients. This casual position will encompass duties related to Case Management, Intake and Single Session Counselling in collaboration with Team Based Care from other disciplines.
