In accordance with the Vision, Purpose, and Values, and strategic direction of the Vancouver Island Health Authority (Island Health), patient and staff safety is a priority and a responsibility shared by everyone; as such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.

Reporting to the Manager, Program Practice and Education, the Clinical Social Work Educator, Primary Care works collaboratively as a member of the inter-professional Primary Care Clinical Support Team (CST) to support Primary Care staff throughout Island Health to attain excellence in clinical practice. Facilitates the development, provision and evaluation of clinical strategies, education and clinical resources relevant to social work practice in Primary Care, in the context of the inter-professional team. Supports Social Workers working in a range of settings including Patient Medical Homes, Urgent and Primary Care Centres, Nurse Practitioner Clinics, and other Primary Care service models. Collaborates with colleagues in other disciplines to ensure appropriate Primary Care clinical practice development .

Applies a broad knowledge base of the health care environment, relevant research and applicable theories (educational, organizational, change management, systems thinking), team-based care approaches and the science and art of Social Work practice to role model, provide leadership and consultation to promote a positive learning culture and advance professional practice.

Travel is a requirement of this position and transportation arrangements must meet the operational requirements of Island Health.

QUALIFICATIONS:Education, Training And ExperienceMasters degree in Social Work from an approved post-secondary institution; and Current full registration with the BC College of Social Workers; and Five years of recent related experience which includes two years of relevant clinical experience; and Knowledge of adult education and learning principles; and Current knowledge of available programs and services, their interrelationships, and their function in delivering care in the Community; and Working knowledge of applicable legislation, including but not limited to the Adult Guardianship Act, Mental Health Act, Health Care (Consent) and Facility (Admission) Act.

An equivalent combination of education, training and experience will be considered.

Valid BC Driver's LicenseSkills And AbilitiesAbility to develop educational strategies and programs for those providing clinical services in Primary Care. Ability to integrate adult education and learning principles into programs and teaching strategies. Demonstrated organizational, problem solving and teaching skills. Demonstrated advanced competence congruent within the organization's framework and standards. Ability to function independently and collaboratively within a team. Effective oral and written communication skills. Effective interpersonal and facilitation skills. Ability to organize, plan, and prioritize. Analytical and decision-making skills. Computer skills, including the ability to use MS Office and other related computer technology. A sound understanding of current and future health care issues in Primary Care and the potential impact on professional education and clinical practice. Ability to operate related equipment. Physical ability to perform the duties of the position.
