In accordance with the Vision, Purpose, and Values, and strategic direction of the Vancouver Island Health Authority (Island Health) patient and staff safety is a priority and a responsibility shared by everyone; as such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.

Under the supervision of the Program Manager or designate, the Ledger Social Worker provides intake support by screening and accepting referrals for a variety of CYFMHS Programs. Working within the context of a patient/family-directed approach, the vision/values of the program, Island Health, and in accordance with established Standards of Practice, including a Trauma Informed Model of Care, the Ledger Social Worker also provides case management services for clients and their families admitted to the Ledger inpatient program and works collaboratively with the inpatient multi-disciplinary team to assess care needs and facilitate discharge planning.

QUALIFICATIONS:Education, Training And Experience

Bachelor of Social Work degree (B.S.W.) from an accredited school of Social Work. Registered with the BC College of Social Workers . Minimum of two (2) years experience with pediatric mental health programs and services. Must possess a strong understanding of relevant community resources and services.

Skills And AbilitiesComprehensive knowledge of local and regional resources for children and youth who have mental health concerns.Ability to communicate effectively both verbally and in writing.Ability to establish and maintain effective working relationships with clients, families, professional colleagues and other service providers.Ability to use professional judgment, empathy, tact, and integrity.Ability to make appropriate assessments in situations where the client and family are under stress.Ability to take and record social histories and provide short-term counseling to families awaiting admission.Ability to counsel in crisis situations.Ability to be flexible and adaptable in a fast-paced and changing environment.Able to work effectively as an active member of a multidisciplinary team.Working knowledge of various computer applications (MS Office, MS Outlook, Excel, Cerner, etc. Link to Full Job Description https://jd.viha.ca/JD3458.pdf Additional information for applicants Additional Clauses This position is eligible for up to a $15,000 signing bonus.
