Reporting to the manager/designate, facilitates student learning and recruitment to Island Health positions, and supports quality improvement initiatives. Integral to bridging client's healthcare journey to self-management in community settings, applies to the role a broad knowledge base of community health care and services environment; adult education theory and models; program/curriculum development and evaluation; organizational, teams and systems development; process consultation and sustainability.
In collaboration with other regional Educator colleagues, holds accountabilities for the establishment, implementation, monitoring and evaluation of falls prevention initiatives that support and improve patient/client care and safety, and align with organizational and accreditation requirements.
Plans, continually develops, and operates a student led outpatient clinic for community dwelling older adults at risk for falling. This position requires the therapist be available when clinical placements are scheduled, thus scheduled leaves will occur outside of clinical placement dates.
Travel is a requirement of this position. Transportation arrangements must meet the operational requirements of Island Health in accordance with the service assignment and require the use of a personal vehicle.
QUALIFICATIONS:Education, Training And ExperienceGraduation from a recognized university program in Physiotherapy.Current licensure in a practicing category with the College of Health and Care Professionals of BC as a Physiotherapist.Five (5) years' recent, related experience in working with older adults with chronic conditions or recent illness/injury, as well as experience with educating and quality improvement.An equivalent combination of training and experience may be considered. Valid BC Driver's License. Skills And AbilitiesAbility to integrate and apply CHCPBC Standards for Physical Therapy Practice in the context of community practice and UBC MPT Clinical Education Manual to facilitate learning experiences appropriate to student's level. Demonstrated ability to support learning through coaching, mentoring and hands on training; 1:1 and in groups. Working knowledge of community health, determinants of health, Expanded Chronic Care Model, health promotion, illness prevention, and the challenges of adults living with complex health conditions. Ability to interact/communicate with people from a wide variety of socio-economic/cultural environments. Demonstrated ability to prepare/maintain a variety of charts, records and reports. Working knowledge of community resources. Demonstrated ability to prepare a plan and execute on deliverables to enact a change. Demonstrated ability to organize work, set objectives and establish priorities. Demonstrated ability to manage time and resources, implement activities to promote cooperation among the interdisciplinary team, and collaborate across disciplines. Ability to demonstrate creative planning for change and innovations, implementation of Island Health policies or other protocols, and ongoing professional development of self and others. Demonstrated ability to foster teamwork and a commitment to excellence in the provision of client care. Physical ability to perform the duties of the position.
