Under the supervision of the Program Manager or designate, the Intake Clinician - Foundry provides intake support by receiving referrals from a variety of sources including mental health teams, hospitals, physicians, family members, community agencies and self-referrals; and determining appropriateness of request and client eligibility, to ensure clients meet Foundry program mandates. The Intake Clinician - Foundry provides case management services for clients and their families referred to Foundry programs, and works collaboratively with the multi-disciplinary team to assess care needs and facilitate planning.
QUALIFICATIONS:Education, Training And ExperienceBaccalaureate Degree in a health related discipline from an approved post-secondary institution One (1) year recent related experience working with youth and young adults with mental health and/or substance use concerns Recognized post basic education related to mental illness and/or substance use An understanding of mental health and substance use concerns; assessments, treatment methodologies, intervention strategies, the change process, and community dynamics; knowledge of applicable legislation, such as Mental Health and Child Protection legislation; knowledge of available programs and services their interrelationships, and their function in delivering services in the community;or an equivalent combination of education, training and experience.
Skills And AbilitiesComprehensive knowledge of local and regional resources for youth Ability to communicate effectively both verbally and in writing. Ability to establish and maintain effective working relationships with clients, families, professional colleagues and other service providers. Ability to use professional judgment, empathy, tact, and integrity. Ability to make appropriate assessments in situations where the client and family are under stress. Ability to take and record social histories and provide short-term counseling Ability to counsel in crisis situations. Ability to be flexible and adaptable in a fast-paced and changing environment. Able to work effectively as an active member of a multidisciplinary team.
