Salary Range: $19.13 - $26.36

Pay rates are determined based on experience and internal equity.

Position Summary:

Under the general supervision of the CCC Administrator, coordinates all referrals for the facility. Maintains all admission applications. Organizes the admission process for all new residents with nursing/admissions nurse/care plans, nursing supervisor, Administrator, Director of Nursing and RNA’s.

Work in shared office. Participate in performance improvement activities  

Position Duties/Responsibility: 

Coordinates SNF admissions from other facilities and LTC admissions from home or other facilities. Works as a liaison between the Director of Nursing, the CCC Admissions Nurse and the Case Managers at the transferring facility. Maintains the resident lists and room changes. Keeps records of applicants contact person and address (mailing list). Keeps the District Admission Team aware of bed changes and admits. Contacts applicants when admission is available and assists families in understanding the admission procedures. Picks up payroll vouchers on pay day and has them available for staff by 0730. Receives visitors and telephone calls. Directs visitors as needed and conducts facility tours on request. Ability to verify insurance coverage, obtaining necessary authorizations for admissions to facility. Ability to communicate with healthcare providers, insurance companies and patients to gather relevant information and ensure compliance with pre-authorization requirements. Maintaining up to date knowledge of insurance policies and procedures is crucial in this role. Accompanies the Administrator/Director of Nursing for home visits for potential admissions. Assists with dedicated events and fund raisers. Ability to pre-screen residents to reduce workload on Admissions Nurse. Other duties assigned by the CCC Administrator.

The ability to work as a Team member of the district. The ability to have positive personal interactions with staff, residents, physicians, and visitors.

Position Qualifications: 

Education: High School graduate or GED.                              Experience:Prefer experience performing clerical tasks.   Certification: None  Skills/Ability: nMust have basic knowledge of Microsoft Word and Exce Demonstrate positive public appearance and effective communication skills  Demonstrate correct spelling, grammar, and punctuation.
