Senior Case Manager The Rescue Mission of Trenton is seeking a Senior Case Manager for our Emergency Shelter Unit. Under the direction of the Case Manager Coordinator, this position will provide direct case management and supportive services to emergency shelter clients with Substance Use Disorders. This position is an important part of helping our clients navigate independent living. Qualifications For Position + Bachelor’s in Social Work, Counseling, Psychology, or related field (Master’s degree preferred). + Minimum of 3-5 years of experience in case management, social services, or substance use treatment, with at least 2 years in a supervisory or senior role + Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC)or Certified Alcohol and Drug Counselor (CADC) preferred + Valid Driver's License Job Responsibilities and Duties + Review assessments/service plans to ensure they are accurate, thorough, and align with goals + Provide guidance on prioritizing client needs and recommend interventions or resources + Oversee implementation of service plans and ensure proper coordination among team + Resolve barriers in accessing services by advocating with external agencies or programs + Provide expertise in complex cases, such as co-occurring disorders or housing challenges + Review documentation and case progress for quality assurance and compliance + Provide mentorship to staff, offer strategies for effective service delivery + Facilitate case conferences to discuss and resolve complex issues or stagnant progress + Lead crisis response efforts, including team coordination and decision making + Evaluate crisis interventions and recommend policy or procedural improvements. + Review and approve discharge plans, ensuring they align with goals and program objectives + Evaluate client outcomes to assess program effectiveness and identify areas of improvement + Lead team meetings, ensuring collaboration and shared understanding of client needs. + Provide training and support to Case Managers and CPRS's fostering professional growth + Monitor program performance, ensure compliance and report outcomes to leadership Salary: $52,500 plus a comprehensive benefit and training packageWork Schedule: 40 hours a week including one weekend day (TBD)The Rescue Mission of Trenton is committed to ensuring that all current and prospective employees are afforded equal opportunities and treatment and a harassment-free work environment.
