About the Job

The Senior Director of Non-Traditional Network Design and Partnerships is a forward-thinking role focused on developing and managing innovative partnerships with non-traditional healthcare providers and community-based organizations (CBOs). This position is responsible for identifying, recruiting, contracting, and managing relationships with organizations that address social determinants of health (SDOH), provide community health services, and offer alternative care models. This role requires a strong understanding of the evolving healthcare landscape, a commitment to addressing health equity, and the ability to build collaborative partnerships with diverse stakeholders. The Sr. Director/AVP of Non-Traditional Network Design and Partnerships will have accountability for advancing CINQCARE’s position in the care in the home market space.

This position reports to the VP, Network Management. The Senior Director of Non-Traditional Network Design and Partnerships should embody CINQCARE's core values, including Trusted, Empathetic, Committed, Humble, Creative, and Community­ Minded.

Primary Responsibilities

The Senior Director of Non-Traditional Network Design and Partnerships will have the following responsibilities: 

Non-Traditional Network Strategy and Development:Develop and implement a strategic national plan for building and expanding non-traditional provider and SDOH networks, aligned with the organizational goal of delivering health and care in the home for our family members.Conduct market research and environmental scans to identify potential non-traditional partners, including but not limited to in-home care specialty and ancillary providers (e.g., dental), CBOs, social service agencies, housing organizations, food banks, transportation providers, and other relevant entities.Develop a clear value proposition for non-traditional providers to participate in the CINQCARE care in the home networks.Provider Recruitment and Contracting:Oversee and manage a team of contractors focused on care in the home provider recruitment.Build out recruitment plans: Identify, oversee recruitment, and onboarding of non-traditional providers into our networks; ensure alignment with network criteria, quality standards, and market specific contractual requirements.Develop innovative contracting models that recognize the unique financial and operational structures of non-traditional providers (e.g., value-based contracts, shared savings arrangements, grant funding collaborations).Negotiate contracts and service agreements with non-traditional providers, ensuring clear performance expectations, data sharing protocols, and compliance with relevant regulations.Relationship Management and Collaboration:Serve as the primary point of contact for non-traditional provider partners within the network.Build and maintain strong, collaborative relationships with key leadership and staff at partner organizations.Facilitate communication and coordination between non-traditional providers and traditional healthcare providers within the network.Develop and implement strategies to integrate non-traditional services into care delivery models and workflows.Program Development and Integration:Collaborate with internal stakeholders (e.g., care management, population health, data analytics) to develop and implement programs that leverage non-traditional provider services to address SDOH and improve health outcomes.Identify opportunities to integrate non-traditional services into existing care pathways and value-based care programs.Develop and implement training programs for both traditional and non-traditional providers to promote effective collaboration and integrated care delivery.Data Analysis and Performance Measurement:Develop metrics to track the impact of non-traditional provider services on health outcomes, cost of care, and patient satisfaction.Collect and analyze data to evaluate program effectiveness and identify areas for improvement.Prepare regular reports for leadership on network performance and program outcomes.   Advocacy and Community Engagement:Represent the organization in community forums, coalitions, and partnerships focused on addressing SDOH and improving community health.Advocate for policies and funding that support the integration of non-traditional providers into the healthcare system.

Qualifications

Bachelor's degree in healthcare administration, public health, social work, business administration, or a related field; Master's degree preferred.Minimum of 5-7 years of experience in healthcare, social services, community development, or a related field, with demonstrated experience working with CBOs or other non-traditional providers.Strong understanding of SDOH, community health, and the social service landscape.Experience in program development, implementation, and evaluation.Excellent communication, interpersonal, and relationship-building skills.Ability to think creatively and develop innovative solutions.Strong project management and organizational skills.

The working environment and physical requirements of the job include:

This position requires both in-home and office-based work. The job requires frequent travel for home visits and travel to physician offices, hospitals, sub-acute facilities, community partners and non-home-based market offices in all types of weather conditions. In-office work is performed indoors in a traditional office setting with conditioned air, artificial light, and an open workspace.

In this position you will need an ability to travel frequently by car and/or public transportation, the ability to communicate with customers, vendors, management, and other co-workers in person and over devices, sometimes with people who are agitated. Regular use of the telephone and e-mail for communication is essential. Sitting for extended periods is common. Must be able to receive ordinary information and to prepare or inspect documents. Lifting of up to 30 lbs. occasionally may be required. Good manual dexterity for the use of common office equipment such as computer terminals, calculator, copiers, and FAX machines. Good reasoning ability is important. Able to understand and utilize management reports, memos, and other documents to conduct business.
