Oak Street Health's impact on the communities we serve is large and growing.  This Senior Medical Director role will put you at the leading edge of the value-based care movement by being a part of our Central Care Model Development team.

 

The Senior Medical Director of Population Health will be responsible for co-leading, with Vice Presidents of the Population Health and Central Services Departments, a range of clinical and strategic initiatives. The Population Health program focuses on the design and implementation of the Oak Street Care Model.  The Oak Street Product team which designs, builds and implements software tools to support care team workflows and meet their information needs is a part of the Population Health team and therefore technology work will be part of the scope of the team and position.  Although the vast majority of Oak Street's care delivery is organized through its Divisions and Regions, there are also centralized clinical efforts which most often reside within the Population Health or Centralized Services Teams.  The Senior Medical Director of Population Health will provide clinical leadership to a portfolio of programs and planning initiatives within this domain and may also supervise other Clinical Leaders performing this work. 

 

Core Senior Medical Director of Population Health Responsibilities:

 

In addition to clinical duties caring for Oak Street patients as a primary care provider 1-2 days per week in one of our Centers, the Senior Medical Director is responsible for the following general leadership and administrative duties (note an addendum may be added to this job description based on the specific focus areas for a given open role):

 

Organizational Strategy and Collaboration: In partnership with other senior Population Health leadership, the Senior Medical Director for Population Health will help lead organizational strategy development for predefined pillars of the Oak Street Health care model. This will include helping determine Strategy and Tactics to achieve organizational goals, vetting new internal and external opportunities, designing programs and clinical workflows, and creating clinical content. The Senior Medical Director will also collaborate closely with Population Health, Central Services, and Divisional / Regional leaders in these efforts.  Additionally, the Senior Medical Director may be asked to participate or lead workgroups or committees.

Population Health program clinical leadership:  This physician or Advanced Practice Provider will be the clinical leader for a set of programs and/or initiatives within the Population Health and Centralized Services Teams.  Most often, the Senior Medical Director will be partnered with a VP level dyad partner focused on operations for each program.  As the clinical leader for the programs, it will be the Senior Medical Director responsibility to assure the clinical services are high quality, patient centered and address Oak Street's broader goals.  Depending on the program design, the Senior Medical Director may be expected to provide direct clinical supervision to RN's, NP's, physicians, and/or other clinical or support staff involved in delivering care through these programs. 

Lead Collaboration with clinical and operational leaders:  The Senior Medical Director will lead bidirectional communications with stakeholders across the organization related to their programs and initiatives.  The Senior Medical Director will be expected to lead the gathering of input from the Medical Directors of our centers, regions and divisions as well as keep leaders across the organization informed of our central care model planning and programmatic efforts.  

Supervise and develop Clinical Leaders: Within the Population Health and Care Programs Teams, the Senior Medical Director may supervise and/or support Medical Directors performing strategic and programmatic work to enhance the impact of their work and develop their strategy, communication, and operations skills.  This could additionally include supervision of center or regional Clinicians and clinical leaders assigned to contribute subject matter expertise to guidelines, clinical decision support, informatics, or other efforts.

What are we looking for?

 

An ideal candidate will satisfy the following;

US work authorization

M.D. / D.O. / APN / PA clinical training including an active license to practice.

Physicians should be Board certified in Internal Medicine, Family Medicine, Preventative Medicine or Geriatrics.

Fellowship training, professional degrees (e.g., M.B.A., J.D., M.P.H.), and/or 2-5 years of management consulting experience.

Training and experience providing primary care is preferred.

1-3 years of clinical leadership or supervision experience is required. 

Experience with managed care and/or value-based practice and familiarity with payer-provider collaboration.

Experience using a metrics-driven approach to analyze cost, quality, and satisfaction data to drive clinical strategy and program redesign.

Experience developing and implementing new processes. 

Understand the basics of managed care 

Ability to collaborate effectively with a staff, providers, and a diverse group of leaders.

Additional qualifications for this role include:

Demonstrated expertise (through certifications and / or experience) in the accurate and appropriate use of ICD-10 codes and HCC documentation
