+ 40 hours per week
+ Monday – Friday
+ Remote work 2 days per week

+ Provides professional social work services in a client/patient clinical teaching and research environment.
+ Provides social work services to clients/patients, their families, and/or others, all while exercising independent decision-making authority and expertise specific to assigned area of practice. **DEPARTMENT DESCRIPTION**
The UW Primary Care team of Social Workers is dedicated to advancing UW Medicine's mission of improving the health of the public. We strive to provide high quality patient-centered care and support the continued learning of our team members.
Located just off of I-90 near the Issaquah Transit Center, the UW Medicine Primary & Urgent Care offers appointments six days a week, including evenings and same-day urgent care.
Board-certified providers offer a complete spectrum of primary care services, including family medicine, internal medicine, behavioral health, travel medicine, nutrition services, and sleep medicine. For your convenience, our site includes an on-site lab and digital X-ray facilities. Clinic providers speak various languages including Spanish, Mandarin Chinese, Portuguese and Hindi.
**PRIMARY JOB RESPONSIBILITIES**
+ Provides specialty consultations among health care staff and health care teams regarding psychosocially focused aspects of client/patient care.
+ Provides psychosocial support and support utilization guidance to patients and families.
+ Works in collaboration with a multidisciplinary team to develop and implement safe effective case management and counseling programs in specialized programs.
+ Provides care coordination for patients with complex social and medical histories.
+ Makes timely referrals to hospital and/community agencies, including those required by mandatory reporting laws such and child and vulnerable adult abuse.
+ Participates in practicum instruction of social work students and provides work direction to Social Work Assistant(s) and other staff.
+ Engages in departmental program development activities, tasks forces, and committees. **REQUIREMENTS**
+ Masters of Social Work Degree from a program accredited by the Council on Social Work EducationAND
+ One year of full-time social work experience in a health care setting or equivalent.(Experience can include a practicum placement as part of a social work training program and/or employment experience.) **WASHINGTON STATE LEGAL REQUIREMENT** Within 90 days of hire must hold one of the following credentials issued by the state of Washington:Agency Affiliated Counselor, or
+ Licensed Social Work Associate - Advanced, or
+ Licensed Social Work Associate - Independent Clinical, or
+ Licensed Advance Social Worker, or
