Sepsis Navigator, RN *Full Time, Days*

Under the direction of the Sepsis Nursing Supervisor at Providence Mission Hospital, the Sepsis Nurse Navigator works with the care team to implement program goals and objectives by providing culturally competent and linguistically appropriate care. 

The Sepsis Nurse Navigator faciliates the care of patients with Limited English Proficiency (LEP) with the diagnosis of sepsis across the heathcare continuum. He/she provides support to LEP patients and their families with a focus on providing education, enhancing the communication with the care team, and facilitating discharge planning and continued follow up and support as needed through 30 days post-discharge. The Sepsis Nurse Navigator collaborates with community health/nurse navigators at the Family Resource Centers (FRCs), community health clinics, community organizations and groups to support and promote the health and wellness of the whole person body, mind and spirit. 

He/She provides health education and health related services including one on one teaching, classes, health advocacy, health access support groups, case management and outreach to identified underserved populations. The Sepsis Nurse Navigator will tailor services to the unique health and wellness needs of the individual/family served. 

Being sensitive to cultural and belief system differences, utilizing good communication skills, the Sepsis Nurse Navigator initiates and cultivates positive working relationships with hospital staff and individuals. She/he is self-directed and able to work independently in meeting program goals and needs of communities. 

Proficient in Spanish language in both verbal and written communication. Successful completion of Bilingual Fluency Assessment for Clinics (BFAC) will be required upon hiring. Self motivated and takes creative initiative in job responsibilities to work in a variety of community situations requiring self direction independent decision making Ability to organize, prioritize and complete and/or delegate activities and assignments in an efficient manner, using work time productively Knowledge of community health nursing principles, possessing analytical skills to asses needs of patients, develop and implement programs and standards that improve the quality of life of patients out in the community Highly functional computer skills in Word, Excel, and others as related to perform data collection, report completion, and as needed and trained by hospital/health ministry staff Ability to take action to resolve independently those issues, problems, and concerns which do not require attention of the Sepsis Clinical Coordinator Understands and promotes wellness and healthy lifestyle, integrating concepts of body, mind, and spirit in its relationship to maintenence of health and prevention of disease  Verbal, written, and electronic communication skills and interpersonal skills that promote dignity and care in the community and with collaborators  Proficient in required competencies upon completion of 90-day orientation

Required Qualifications:

Bachelor's Degree In health related field. Preferred field: Nursing or Public Health Nursing. Or Minimum of 3 years of acute care experience. Upon hire: California Registered Nurse License  Upon hire: National Provider BLS - American Heart Association 3 years Acute care experience. 1 year Experience working with patients with Limited English Proficiency (LEP).

Preferred Qualifications:

Coursework/Training in Case management. Expanded experience in working with patients from different cultures and with different levels of education. Experience in Public Health Nursing, Home Health Nursing, Community Health Setting, or in a Health Education setting. Experience in working with variety of community groups/organization, education institutions or faith based communities. Experience in working with variety of community groups/organization, education institutions or faith based communities. Some supervisory experience.
