Service Coordinator

osition Summary:

The Service Coordinator acts as the primary contact for all Youth Pathways participants and is responsible for following the participant throughout their time in the program, follow-up services post-program, and ensuring they are connected to and receiving all appropriate services. The candidate should have a background in case management and social services, and a willingness and desire to work with at-risk and vulnerable populations. They will reflect the Fedcap values of teamwork, collaboration, and innovation.

 The Service Coordinator is responsible for acting as the main point person for participants currently or formerly enrolled in the Youth Pathways program. The Service Coordinator is responsible for providing services to each participant on his or her caseload, including ensuring a proper service plan that includes participants short and long-term goals, the connection to proper resources including education, training, job readiness, soft skills, and essential needs such as food and housing, and proper follow up with participants after they have left the program due to enrollment in job search, education, or obtaining employment. 

 

Essential Job Functions:

Work collaboratively with participants to determine the enrollment of services and activities based on the assessment of the participant’s employment and educational goals, preferred location of service, educational and skill levels, potential barriers, and the service coordinator's own observation.  Address any immediate barriers that may prevent the participant from enrolling in appropriate services or activities.  Continually evaluate and advocate for the appropriate services necessary to benefit the participant towards his or her goal achievement.  Guide participants to make appropriate, effective decisions in support of their goals. Adapt recommendations and case strategy based on current understanding of participants’ perspective, background, needs, etc.  Participate in the development, implementation, and monitoring of the Service Plan.  Responsible for scheduling and meeting with participants regularly to review progress. Schedule participants for internal activities while enrolled in the program. Follow-up with participants post program to monitor progress towards goals and address any potential barriers. Complete all administrative work such as entering information, such as case notes, into various databases.  Performs other related duties/projects as assigned. Education and Experience:

Bachelor’s degree in human services, Social Work, Education, or comparable work-related experience At least 2 years of experience in case management or service delivery Experience and/or a demonstrated desire to work with the economically disadvantaged youth (18-24), homeless, justice involved, and/or public assistance recipient populations. Excellent communication skills Excellent data entry skills and attention to detail  Proficient in MS Office, including Word and Excel.
