Social Service Case Aide Brooklyn, NY (http://maps.google.com/maps?q=592+Rockaway+Avenue+Brooklyn+NY+USA+11212) Description The Social Service Case Aide provides administrative support to the clinical staff within the department of Behavioral Health services. They provide direct non-billable patient care and program support. Supervisory Responsibilities: · No supervisory responsibilities required. Essential Functions and Duties: · Process necessary documents required for non-billable patient services such as entitlement programs, homecare and transportation forms. · Provide aid to Behavioral Health clinical staff by managing clinical panels by confirming appointments and tracking clinical flow throughout the day. · Provide Behavioral Health screenings for depression, anxiety, tobacco, alcohol, and substances. · Assist with scheduling mental health appointments and follow-ups. · Prepares, records, and transmits information, i.e., demographics and medical social data from patient/client including completing forms and other support documents. · Develop community resources list, and provides para-professional assistance with meeting patient’s needs. · Assist with setting patients up for telehealth visits. · Assist with general program day to day activities. · Provides other related duties as agreed. Requirements Required Skills/Abilities: · Willing and able to work with individuals with mental health needs. · Ability to prioritize and delegate multiple tasks and projects to ensure each is completed correctly and in a timely manner. · Proficient in Microsoft Office. · Proficient in utilizing Electronic Health Records. · Able to establish and maintain effective and collaborative working relationships and demonstrate compassion, respect, courtesy, and tact when interacting with others. · Actively listen, elicit needed information, and communicate information effectively and accurately. · Make appropriate and timely decisions in stressful situations and maintain objectivity. · Ability to maintain confidentiality. Education and Experience: · Associate degree (Human Service, Liberal Arts) or previous experience as a Patient Care Associate or Patient Registration with three years direct patient care experience preferred. Physical Requirements: · Prolonged periods sitting at a desk and working on a computer. · Must be able to lift to 10 pounds at times. · Must be able to traverse all facilities in all types of weather. Salary Description $45,000.00
