This role would assist the Social Service department in meeting the needs of residents and families through various tasks and activities all completed under the supervision of the Soical Service Director. Job Responsibilities: + Assists with coordination of admissions as needed and delegated by Director of Social Services: + Assists with pre-admission counseling, informattion, referral, and tours + Facilitates the intake process + Facilitates PAS screening and Level II + Coordinates admission process with resident and resident representative, community providers and staff + Assures sompletion of admission paperwork + Informs residents of financial responsibilities, payment options and agency contacts according to the federal regulations + Builds and maintains relationships with referral sources; and assists with marketing and outreach + Coordinates room change process + Verifies necessary payer source(s) + Tracks pay or renewal dates + Assists with completion of pay or paperwork renewals and new applications as necessary and/or follows up with appropriate resident representative to ensure necessary paperwork is completed + Acts as facility liaison with counties about pay or renewals and applications + Assists to gather and send in necessary documents to the county + Follows-up with discharged patients who are/were Medicaid Pending to ensure necessary steps are followed so facility can collect/back-bill insurance + Assists with paperwork for guardianship, conservatorship, and/or rep payee + Assists Social Work staff with caseload assignments: + Coordinates and facilitates care conference + Completes necessary social history and Social Service forms and documentation + Assesses pyschosocial changes and identifies strategies to address those changes + Maintains records and documents as indicated and required + Completes MDS and associated assesments per regulations + May assist resident and resident's representative in changing primary care providers + Advoates for resident right and needs + Assists with Advance Directive planning conversations and documentation + Assists with investigations and follow up on grievances + Assists with on-to-one and group counseling as appropriate: + Coordinates Resident and Family Council + Advocates for residents + Provides information and referral to community service providers + Assists with discharge planning as appropriate: + Facilitates discharge plans for resident + Arranges in home and community-based services + Ensures that residents and their representatives are informed of the IDT's recommendations for discharge + Educates residents and their representatives about resources available in the community + Provides referrals to other agencies as needed + Assists in Vulnerable Adult investigations and completes necessary documentation and reports as appropriate: + Completes corresponding reports as appropriate + All other duties as assigned and directed Required: + 0-1 years of experience Preferred: + Bachelor's Degree - BSW from an accredited school of social work + 1-3 years of experience + Two years' experience in LTC + Experience in Medical Social Work **Benefit Overview** Ebenezer offers a generous benefits package, including but not limited to medical, dental, vision plans, life insurance, short-term and long-term disability insurance, PTO and Sick and Safe Time, tuition reimbursement, retirement, early access to earned wages, and more!
