Hourly Pay Rate: $28-$28.48
ESSENTIAL FUNCTIONS/RESPONSIBILITIES
Maintains an understanding and working knowledge of the ICF regulations. Maintains an understanding and working knowledge of agency policies and practices. Ensures each persons’ active treatment program is integrated, coordinated, and monitored. Ensures person-centered planning is integrated, including supporting the person for self-advocacy. Coordinates care and treatment with the ICF clinicians & site team to ensure appropriate and quality services for each person. Develop and maintain Social Services records to include: Social Summary section for each individual’s CFA with annual and as needed updates; quarterly or as needed clinical notes and Social Assessments. Participate monthly in individual program planning with interdisciplinary treatment team (as a member). Participate in annual CFA meetings. Participates in 6 month review meetings as needed. Consult with ICF Program Director and Program Management. Work as a liaison to community resources, including day programs, workshops, hospitals and other social service agencies. Acts as an advocate and ensures implementation of individuals’ rights, policies and fosters dignity and personal worth of each individual. Interprets program services to families and provides other support services to families related to ICF program. Maintains contact with families in conjunction with the site management. Completes admission and discharge paperwork for ICF residents. Provides assistance with individual benefit information and billing as needed. Ensures continued eligibility for benefits such as Social Security, SSI, Medicaid and Medicare. Act as representative for individuals receiving benefits, including completion of representative payee reports, disability update reports, and application and recertification of benefits. Monitor individuals’ finances on an annual basis, or more as needed. Provides follow up and referral to appropriate agencies for individuals who leave the ICF program. Complies with all agency policies and procedures. Other duties as assigned.
MINIMUM QUALIFICATIONS
Bachelor’s Degree in a related field and 3-5 years of experience Valid New York State Drivers License that meets agency policy.
