Social Services Complex Case Manager

Work Schedule: 

100% FTE, Days. Monday through Friday between the hours of 8:00 am and 5:00 pm. This is a hybrid role. When in person you will work at 2275 Deming Way in Middleton, WI

We are seeking a Social Services Complex Case Manager to:

Provide case management for patients with complex psychosocial needs and facilitate solutions to the resource needs, behavioral health needs, and psychosocial/environmental issues of patients and families. Collaborate with internal and external multi-disciplinary teams. Engage patients with the health system and assist team members, patient, and family in overcoming barriers.
