Visit https://www.dignityhealth.org/socal/locations/stmarymedical for more information. **Responsibilities** Under supervision of a LCSW, Social Services Coordinator provides for the psychosocial needs of OB Clinic outpatients with low to moderately complex requirements, Conducts patient assessments recommends plans of care and evaluates results. Performs evaluation of patients and assessments of family needs. Provides crisis intervention, individual, couples, and family therapy with special attention to the psychosocial effects of pregnancy, substance abuse, homelessness, illness and disability. Functions as a member of an interdisciplinary healthcare team. Mentors CPHWs as well as coordinates and maintains up dates and/or changes for the CPSP program. Serves as and auditor for CPHWs chart entries. Serves as a social services resource to the medical team, clinic staff and community. **Qualifications** + 2 yrs in the field of Maternal/Child social work. + Bachelor of Science in Social Work + Excellent organizational skills + Capable of working independently + Working knowledge of clinical and community resources + Computer Skills + Case management crisis intervention + Short-term counseling experience + Bi-lingual (English/Spanish) preferred. **Pay Range** $27.95 - $33.23 /hour We are an equal opportunity/affirmative action employer.
