Social Services Coordinator - Chafee Independent Living

Nexus-PATH is hiring for a full-time CHAFEE INDEPENDENT LIVING COORDINATOR at our Minot, ND location!

Through the Chaffee Independent Living Program, the Coordinator assesses, develops and conducts skills training for eligible individuals, works with community partners to provide services and opportunities, and advocates for resources to assist foster youth ages 16 and older in successfully transitioning youth to adulthood.

RESPONSIBILITIES:

Provides comprehensive coordination of all tasks required to meet the needs and best interest of service recipients consistent with DHS, COA, PATH mission, values, policies, ethics and practice.

Administer assessments and develop plans. Verify eligibility for Chafee Funds. Prepare an annual plan and budget for your region’s Chafee Program. Assist eligible young adults with the ETV process. Utilize the wraparound process in developing, implementing and monitoring the Independent Living plan for the service recipient. Identify and document available match dollars. Facilitate monthly youth group meeting on independent living.

Ensure that documentation is thorough and timely as required by standards.

Provide timely data, quarterly, and year end reports to state coordinator.

Promote the needs of service recipients to ensure successful outcomes.

Advocate for foster/former foster youth; encourage youth to be self-advocates and leaders. Advocate legal rights for service recipients. Advocate for resources to assist service recipients to successfully achieve Independent Living goals. Educate the community about the needs of service recipients. Advocate for the development and maintenance of funding or services to meet youth and family needs at local, state and national levels.

Represent PATH’s mission and values with integrity in all interactions.

Develop a good working relationship with eligible youth and young adults, their caregivers, custodians, informal supports, and social workers, using a positive youth development and wraparound approach. Include input from foster/former foster youth in program design and implementation.

Maintain supervision over the individuals and groups.

QUALIFICATIONS:

BA degree in psychology, vocational rehabilitation, education, criminal justice, child development, or other behavioral field required. Two (2) or more years of experience preferred working with youth in foster care and/or behaviorally challenged youth, and/or emotionally disturbed youth, and/or youth involved in the juvenile justice system, and/or youth within the educational setting. Proof of valid driver’s license, auto insurance and access to vehicle capable of safely transporting clients. Successful completion of background checks and all applicable health requirements. Flexibility to work varied hours, as needed to meet the needs of those served Must be 21 years of age
