Role Overview
As the Social Services Director, you will oversee the social and emotional well-being of our residents. You will play a key role in helping residents and their families navigate their journey in long-term care while ensuring compliance with all applicable regulations.
Key Responsibilities
+ Orient residents and families to the nursing center, providing support during the adjustment period.
+ Complete social histories and MDS assessments and develop care plans that identify medically related social and emotional needs, with actionable goals.
+ Conduct and participate in resident care conferences with residents, families, and interdisciplinary staff.
+ Advocate for and promote resident rights, ensuring a safe and supportive environment.
+ Receive and address resident and family complaints, forwarding them to appropriate staff for resolution and documenting outcomes.
+ Manage the discharge process, ensuring a smooth transition and coordinating with external providers as needed.
+ Perform other related duties as assigned by the facility.
Qualifications
+ Bachelor’s degree from an accredited school in Social Work or a related field (preferred).
+ Minimum of 2 years of experience in a long-term care or skilled nursing facility (required).
+ A physical examination or evidence from a physician indicating freedom from communicable disease (required upon employment).
+ Strong knowledge of social services practices, procedures, laws, and regulations governing long-term care facilities.
+ Must be in good physical and mental health.
+ Proven ability to build rapport with residents, families, and staff.
+ Excellent organizational, communication, and advocacy skills.
