The Social Services Specialist responsibilities include:

Being a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the communityProvides coordination, referral and interdisciplinary discharge planningThey are knowledgeable about treatment, social and economic implications for individuals and families as it pertains to their age, stage of development and discharge needs Possesses a clear understanding of discharge and transition planning and linkage with community resourcesDemonstrates skills in planning, organizing, and managing multiple functions

Location: Bartow Regional Medical Center - Bartow, FLStatus:  Part Time, 16 hours per weekScheduled: Saturday & Sunday 8:00 AM - 4:30 PMWeekend Requirement:  EveryOn Call: No

 

Certifications and Licensures:

Preferred ACM (Case Management) orPreferred CCM (Case Manager)

 

Education:

Required Bachelors in Social Work, Sociology, or Related Field

 

Experience:

Required 6 months Internship orRequired 6 months Social Work in an acute care setting
