(https://hr.uw.edu/benefits/wp-content/uploads/sites/3/2018/02/benefits-summary-classified-staff-greater-than-half-time-20230701\_a11y.pdf) **HARBORVIEW MEDICAL CENTER SOCIAL WORK** has an outstanding opportunity available for a **SOCIAL WORK ASSISTANT - INPATIENT** . **WORK SCHEDULE** • Full-Time / 40 hours per week • Day Shift **POSITION HIGHLIGHTS** • Provide care coordination to help effect early discharge placement, and serves as a liaison between Skilled Nursing Facilities, Adult Family homes, transportation brokers, outside community agencies etc. as delegated by the Inpatient Social Work Supervisor • As needed, this position will assist in the Bed Readiness program to facilitate patient discharge and data collection **PRIMARY JOB RESPONSIBILITIES:** • Provide services to HMC patients while adhering to the parameters of HMC policies and procedures and NASW Code of Ethics • Maintain working knowledge of basic medical conditions, terminology and community resources • Serve as an ambassador for the Social Work Department and promote effective working relationships • Review incoming referrals for facility placement and prepare/submit electronic referrals for provider review through multiple electronic medical records and referral database • Contact potential providers for admission decisions and inform primary MSW as needed • Maintain working knowledge of available community resources for continuing care options for medically and/or psychiatrically complex patients, to include Skilled Nursing Facilities, Intensive Inpatient Rehabilitation programs, and Adult Family Homes • As needed, coordinate transportation for Bed Readiness patients with skilled nursing facility • Have basic knowledge of DSHS and Medicare benefit programs, eligibility requirements, relevant rules, and regulations • Provide notarial services for patients related to their medical admission or discharge planning. **REQUIRED POSITION QUALIFICATIONS:** Bachelor of Social Work or a Bachelor’s Degree in the behavioral sciences AND two years experience as a Social Work Assistant I or equivalent.
