Social Work Case Manager Inpatient
Overview Expected pay for this position is $28.44 - $33.46/hour. Actual pay will be determined by experience, skills and internal equity. POSITION SUMMARY: The Social Worker Case Manager Inpatient is responsible for identifying, assessing, planning and coordinating, implementing, monitoring and evaluating options and services across the continuum of care for individuals in assigned case load. Health care services are coordinated on behalf of the patient to promote the delivery of care at the appropriate level of care which will optimize clinical outcomes and satisfaction with services. The case manager assures the designation of primary responsibility among team members to each aspect of the plan, avoiding duplication and fragmentation. The case manager assesses and intervenes to address the complex psychosocial and behavioral health issues and needs associated with hospitalizations and transition plans. The case manager coordinates with the health care team to facilitate and expedite the best outcomes for the patient and physician caring for that patient. Demonstrates the knowledge and skills necessary to provide services to the assigned patient population. The case manager works with the patient and treatment team to help establish goals and realize outcomes as determined by current individualized care plans. Works with the Care Management Division leadership team to ensure alignment with the strategic goals and visions. Qualifications REQUIRED QUALIFICATIONS: Education: Bachelor’s degree in Social Work (BSW) from a school or program accredited by the Council on Social Work Education Licensure/Certifications: Shall comply with the Illinois state licensing and certification requirements or jurisdiction(s) in which he or she practices. Social Worker with a BSW who has not obtained 3 years of supervised clinical hours will be given the opportunity to complete this clinical practicum required to sit for the examination/apply for licensing for practice of social work. All BSW Mission Partners must obtain applicable license as awarded by the Department of Professional Regulation within the mission partner's designated state (Illinois/Michigan) of practice within 4 years from date of hire with detail timeline below. Mission Partner is required to identify clinical supervisor by day 91 of hire with a plan to complete the supervision within in 3 years Mission partner is accountable to ensure completion of state application form Once the 3 year clinical supervision is completed the mission partner must submit state application within one month All BSW mission partners must obtain licensure (LCSW or LCPC) within 4 years from date of hire If a Mission Partner comes in with documented supervised clinical hours those will be deducted from the 4 year time requirement Social Worker with an MSW who has not obtained their license will be required to obtain applicable license as awarded by the Department of Professional Regulation within the Mission Partner's designated state (Illinois/Michigan) of practice within 1 year from date of hire with detail timeline below. Must start licensing application process by day 91 of hire All MSW mission partners must obtain licensure (LCSW or LCPC) within 1 year from date of hire Experience: 3 years of supervised clinical hours Broad knowledge of chronic illness and medical and community resources. Positive, empathetic approach and ability to communicate effectively to patients/families with a variety of health issues, age groups and socioeconomic backgrounds. Other skills/knowledge: Excellent interpersonal and communication skills. Solid computer skills, including proficiency with Microsoft software. Strong analytical and problem solving skills, with the ability to be detail oriented. PREFERRED QUALIFICATIONS: Education: BSW or Master degree with licensing in Social Work from a school or program accredited by the Council on Social Work Education Master degree in social work Experience: 3 years of experience caring for patients with progressively increasing responsibility. Chronic, complex illnesses adult and/or pediatric care management experience preferred. Other skills/knowledge: Care Management certification for adult or pediatric as appropriate within 3 years of hire. Mission partner must take exam by 3rd anniversary, if you are unable to successfully pass this certification exam this deadline can be extended by one year.
