Join East Boston Neighborhood Health Center’s PACE program where the Social Work Participant Navigator role is engaging with every day offering exciting new challenges. As part of a dynamic Interdisciplinary Team (IDT) in an innovative model of care, this position will act as case manager and support for the Neighborhood PACE Social Work department and PACE participants. Work with older adult participants and families to complete case management and housing tasks to maximize participant and caregiver satisfaction. Participate on an interdisciplinary team with the goal of supporting elders with complex care needs to remain in the community. An ideal candidate will be knowledgeable about community resources, be detail oriented, able to multitask, and possess excellent communication and computer skills. Must hold a Bachelor’s degree in social work or related field or have two years of relevant work experience in a community setting. Bilingual (English/Spanish) preferred. Automobile required.BS or BA in social work or equivalent work experience in a community settingOne year work experience with the elderly preferably in an interdisciplinary team settingFlexibility, creativity and good customer service skills.The ability and willingness to assist outside of traditional roles and with many disciplines. Requires travel between the PACE centers and for home visits as needed.
