40Gundersen Health System (becoming Emplify Health) is looking for a full-time Social Worker, Complex Discharge Planner! This highly skilled Social Worker would have additional time to allow for increased collaboration with post-acute care services, county and/or legal resources, physicians, nursing, and other interdisciplinary team members within the organization.
What you will work: 1.0 FTE = 80 hours every 2 weeks Monday - Friday, Days One holiday per yearWhat you will do: Ensure cooperation and coordination on a regular basis between all key stakeholders for their patients to allow for clear, concise, and timely discharge planning to occur.Assessment and treatment planningDischarge planningProvide patient and family education and support Documentation and communication - Document all assessments, interventions, and services in the electronic health record.
