Position purpose:
We are seeking a full-time social worker to serve as part of our team on the Flourish Mobile Health Clinic. This person will provide trauma-informed, harm reduction-oriented social work services to vulnerable populations, including individuals affected by substance use disorders (SUD), homelessness, and limited access to healthcare. This position supports the clinic's interdisciplinary care model by providing case management and care coordination including but not limited to coordinating behavioral health referrals, providing crisis intervention, connecting patients with community resources, engaging in community outreach, and mentoring students from various health disciplines.
Essential Functions:
Direct patient care & support by assisting with providing screenings, brief interventions, and harm reduction to patients, in addition to maintaining accurate and confidential patient records and documentation. Case management & care coordination through development of care plans, coordinating referrals and follow-up, and assisting with required application submissions for community resources to meet individual needs. Collaborate with faculty to supervise students to enhance clinical education and provide hands-on learning experiences and field experiences for students. Respond to emergency situations which includes mental health, such as crisis intervention.Engage and build relationships with community partners to decrease barriers to care for patients.
